Role OverviewJoin JourneyCare Hospice as a Hospice RN Case Manager, working Monday-Friday with one weekend every 12 weeks and one holiday. The role involves patient and family caseload management, assessment, and care planning.
What You Will Do
Assume primary responsibility for patient and family caseload, including assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process. Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors.

Requirements
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Must possess current CPR, First Aid Certification
- Must be knowledgeable of quality assessment and assurance procedures
- Valid driver’s license and proof of insurance is required
