Role OverviewThe Hospice RN Case Manager will serve as Serenity Hospice's primary clinical presence at a West Chester facility, overseeing the hospice care of patients residing there. The ideal candidate will build strong relationships with patients, families, and facility staff, and become a trusted clinical leader and resource within the facility.
What You Will Do
Manage and coordinate individualized hospice plans of care for patients, perform comprehensive nursing assessments, and provide compassionate nursing care focused on comfort, dignity, and quality of life.

Requirements
- Current, unrestricted Pennsylvania RN license
- Strong hospice nursing experience
- Solid knowledge of hospice philosophy, symptom management, and end-of-life care
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal skills
- Ability to communicate effectively and compassionately with families during difficult and emotional situations
- Ability to build strong professional relationships with facility staff and leadership
- Strong organizational and time-management skills
- Confidence working independently and making sound clinical decisions
- Strong leadership skills and the ability to take ownership of the facility partnership
- Familiarity with hospice EMR documentation and regulatory requirements
- Valid driver's license and reliable transportation
