As a Hospice RN Case Manager, you will serve as the primary nurse for your assigned patients and families, providing compassionate care and guidance throughout their hospice journey.
What You Will Do
Responsibilities include managing and coordinating patient care plans, completing comprehensive patient assessments, leading hospice admissions and care planning, collaborating with physicians and interdisciplinary care teams, assessing and addressing physical, emotional, social, and spiritual needs, and providing support and education to patients and families.

Requirements
- Graduate of an accredited Registered Nursing program
- Active RN license in the state of Ohio
- Current CPR and First Aid certification
- Strong clinical assessment and decision-making skills
- Excellent communication and relationship-building abilities
- Passion for hospice and patient-centered care
- Valid driver's license and reliable insured transportation
