Role OverviewAs a Hospice RN Field Nurse, you will provide high-quality care to patients in their own homes, empowering them to maintain their independence and dignity. You will work with a team of healthcare professionals to plan, organize, and direct home care services, emphasizing community health education and experience.
What You Will Do
Your primary responsibilities will include completing patient assessments, supervising LPNs and Home Health Aides, developing patient education plans, and evaluating and revising care plans as patient needs change.

Requirements
- Graduate of an accredited school of nursing.
- Active RN licensure
- One (1) to two (2) years of recent acute care experience in an institutional setting
- Bachelor's degree with one (1) year of home health care experience preferred
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
