Role Overview

As a Hospice RN, you will be a clinical leader, delivering high-quality, personalized nursing care in a patient's most comfortable environment. You will craft individualized care plans, collaborate with a multidisciplinary team, and empower patients and their families through education and emotional support.

What You Will Do

Your day-to-day responsibilities will include developing and implementing comprehensive care plans, collaborating with a team, and mentoring and guiding LPNs, LVNs, and Home Health Aides. You will also utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.

Requirements

  • Graduate of an accredited nursing program with an active RN license in the state served.
  • Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
  • Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
  • Current CPR/BLS certification.
  • Valid driver’s license and reliable transportation.
