Hospice Social Worker

The Hospice Social Worker is responsible for providing psychosocial services to terminally ill patients and their families. The role involves conducting initial comprehensive psychosocial and bereavement risk assessments, providing psychosocial and anticipatory grief counseling, and assisting with discharge planning.

Requirements

• Master's degree in social work from a school of social work accredited by the Council on Social Work Education
• Valid driver's license and reliable transportation that is insured in accordance with Ohio requirements
• Active Ohio Social Work license
• Minimum of 1 year experience in a health care field with experience working with patients and families coping with terminal illness and dying
