Role OverviewJoin our team as a Hospice Social Worker and provide psychosocial assessments to patients and families, ensuring identified needs are addressed in the patient's plan of care. Demonstrate exceptional service to patients, families, referral sources, co-workers, and other customers.
What You Will Do
Perform psychosocial assessments, counsel patients and families, provide documentation of patient services, assist with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver, and serve as a liaison between patients and families/caregivers and community agencies.

Requirements
- Bachelor's degree in Social Work; Master's degree preferred
- Current State Licensure in Social Work (LSW or LCSW)
- Minimum of one year's experience in health care or hospice setting
- Valid driver's license and proof of insurance
