Role OverviewThe Hospice Social Worker will collaborate with an interdisciplinary team to provide compassionate psychosocial support, advocate for patients and families, and help develop individualized plans of care throughout the hospice journey.
What You Will Do
Conduct comprehensive bio-psychosocial assessments, provide emotional support and education, advocate for patients, and coordinate care with the interdisciplinary hospice team.

Requirements
- Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE)
- Current Montana social work license (LSW or LCSW) or actively working toward licensure
- Minimum of one year of social work experience in a healthcare setting required
- Hospice, home health, palliative care, or community-based care experience preferred
- Knowledge of chronic disease management, end-of-life care, and psychosocial support services preferred
