What You Will Do
Perform psychosocial assessments of patients and families/caregivers, provide counseling and assistance with applications for benefits, and serve as liaison between patients and community agencies.

Requirements
- Social Work Degree
- Texas: Bachelor’s degree in Social Work; Master’s degree preferred.
- Current State Licensure in Social Work
- Texas: LBSW, LMSW, or LCSW
- Minimum of one year’s experience in health care or hospice setting
- Valid driver’s license and proof of insurance
