Job Description
Care Initiatives is hiring a Hospice Spiritual Care and Bereavement Counselor to provide comprehensive care to patients and their families in their final stages. The role requires a Master of Divinity Degree or a Baccalaureate degree in social work, psychology, or a related field, and involves providing counseling and care to meet the spiritual and bereavement needs of patients and families.

Requirements

  • Arrange for counseling or provide counseling to meet the spiritual needs of patients/families/caregivers
  • Provide comprehensive assessment of the patient, family, and caregiver's spiritual needs
  • Identify appropriate spiritual and bereavement interventions and goals for the patient's care plan
  • Participate as a member of the interdisciplinary team (IDT) in the development of patient care plan
