Job Description
The Hospice Team Manager is responsible for the overall on-boarding process for patients admitted to hospice. This includes verifying admission information, managing a team of support staff, ensuring accurate communication, and coordinating documentation for Medicare and Medicaid Recertification. The role involves liaisoning with referrers and providing updates to patients.
Requirements
Current NYS RN License
Current Driver's License
Baccalaureate Degree in Nursing from an accredited school (preferred)
Minimum of 7-10 years’ experience in nursing
Minimum of 5 years of hospice experience
Management experience preferred
Working knowledge of hospice regulations
