Job Description
The Hospice Team Manager is responsible for the overall on-boarding process for patients admitted to hospice. This includes verifying admission information, managing a team of support staff, ensuring accurate communication, and coordinating documentation for Medicare and Medicaid Recertification. The role involves liaisoning with referrers and providing updates to patients.

Requirements

  • Current NYS RN License
  • Current Driver's License
  • Baccalaureate Degree in Nursing from an accredited school (preferred)
  • Minimum of 7-10 years’ experience in nursing
  • Minimum of 5 years of hospice experience
  • Management experience preferred
  • Working knowledge of hospice regulations
