- Doctor Of Osteopathy and Successful completion of Residency program, upon hire
- Doctor of Medicine: NV, upon hire
- Doctor Osteopathic Medicine: NV, upon hire
- Basic Life Support - CPR, upon hire
- Drug Enforcement Administration-Control Sub Reg Cert: NV, upon hire
- Controlled Substance Act:NV, within 120 Days
- Active or eligible license in the state of Nevada State medical licensure
