Role OverviewThe Housing Case Manager supports Plymouth residents by working as part of an onsite permanent supportive housing (PSH) team. The role involves supporting residents in their housing stability, creating Housing Service Plans, and providing care coordination and supportive services. The position also ensures a staff presence in buildings and fosters a sense of community.
What You Will Do
The Housing Case Manager will support residents in scheduling appointments, arranging transportation, and accompanying them to appointments as needed. They will also advocate, consult, and collaborate with community providers to ensure continuity of care. Additionally, they will mediate conflict, intervene in crises, and assist with resident complaints and concerns.

Requirements
- BA/BS degree in a human service field
- Minimum of two (2) years of experience working with homeless and low-income individuals
- Valid Washington State Driver License and insurable driving record
- Excellent written and verbal communication skills

- Ability to develop and sustain successful relationships with tenants and community agencies
- Empathic patience with individuals who are heavily impacted by addiction, mental health concerns, and trauma
- Ability to work effectively under stressful conditions with people of varying socioeconomic backgrounds, sexual orientations, and ethnic diversity
