Job Description
Housing Case Manager responsible for providing intake, psychosocial and substance abuse assessments, and developing treatment plans for clients in assigned caseload. Conducts home visits, coordinates medical and psychiatric services, and maintains case notes in AWARDS/CARES system.

Requirements

  • Bachelor’s Degree in Social Work or related field of experience required
  • 1-2 years of experience working with homeless mentally ill adults or other special needs population preferred
  • Excellent oral, writing and listening skills
  • Proficiency in computer required
