Role Overview

The Housing Navigator provides intensive, wraparound, field-based case management services to unhoused individuals with severe mental health problems, with a focus on securing long-term housing. The role involves connecting clients to permanent housing resources, assessing their housing needs, and developing individualized housing plans. The Housing Navigator also serves as a member of a multidisciplinary treatment team, actively engaging clients in services and supporting their progress towards meaningful recovery goals.

What You Will Do

The Housing Navigator will connect clients to permanent housing resources, assess their housing needs, and develop individualized housing plans. They will also assist clients with linkage to applications for housing-related services, support clients with understanding available resources, and prepare them for interviews with prospective owners/property managers. The Housing Navigator will identify client strengths and barriers to housing readiness/retention and assist participants to reduce barriers and link to resources and services.

They will also have experience using the Coordinated Entry System (CES) and be able to teach prosocial and recovery skills for long-term stability in the community. The Housing Navigator will apply a racial equity lens to all elements of clinical care and collaborate with external partners to ensure high-quality, integrated care.

Requirements

  • BA or BS in any field
  • at least 1 year of case management experience
  • Valid CA driver's license
  • proof of insurance
  • own vehicle required
  • Ability to transport clients in personal vehicle as needed
  • Must not be on any type of legal supervision (probation or parole)
