Job Description
The State Hospital Discharge Initiative Case Coordinator serves as a Behavioral Health consultant supporting older adults transitioning from state psychiatric hospitals to licensed Assisted Living Facilities (ALFs). This role focuses on care coordination, compliance monitoring, and ensuring safe and effective transitions into community-based settings.

Requirements

  • Coordinate referrals and admissions to Assisted Living Facilities (ALFs) for individuals discharging or diverting from state hospitals
  • Conduct virtual and in-person care coordination visits to monitor health, safety, and behavioral health needs
  • Support care transitions between hospitals, ALFs, and community-based settings
  • Monitor participating ALFs for compliance with federal Home and Community-Based Services (HCBS) requirements and state licensing standards
  • Conduct annual audits and complete required HCBS documentation and reporting
