Role OverviewThe ICMS Case Manager provides participant-centered intensive case management, builds ongoing relationships with participants, and coordinates services across Care Campus and community providers to support housing stability, health, wellness, and continuity of care.
What You Will Do
Complete participant intake, assessment, enrollment, individualized service planning, and ongoing case-management follow-up. Connect participants to housing resources, healthcare, behavioral health, substance-use services, benefits, legal resources, and other supports.

Requirements
- Bachelor’s degree in social work, psychology, sociology, human services, public health, or a related field
- Experience in case management, homeless services, behavioral health, housing, healthcare navigation, outreach, or related direct services
- Experience navigating housing, CES, benefits, healthcare, behavioral health, and community-service systems
