Role OverviewIn-Home Assessment Nurse (RN) to complete in-home nursing assessments for individuals receiving services through Georgia Medicaid Waiver Programs and for private-pay care management clients throughout the Central Savannah River Area (CSRA).
What You Will Do
Conduct scheduled nursing assessments in participants' homes throughout the CSRA, complete comprehensive assessments of the participant's current health, functional abilities, safety, and care needs.

Requirements
- Current and unrestricted Registered Nurse (RN) license
- Eligible to practice nursing in the State of Georgia
- Current CPR/BLS certification, if required for assigned services
- Valid driver's license
- Reliable transportation and ability to travel throughout the CSRA
- Ability to work independently in community settings
- Strong assessment, documentation, communication, and organizational skills
- Ability to complete electronic documentation
- Ability to maintain participant confidentiality and comply with HIPAA requirements
