Role OverviewThe Individual Supports Caseworker provides assessment and/or teaching of community living skills to developmentally delayed persons, facilitating their ability to remain in their current home environment. This role involves delivering direct training to individuals in compliance with Agency policy and procedures. The Individual Supports Caseworker reports to the Director of Day and Community Services and is compensated on an hourly basis.
What You Will Do
Provide home visits to assess new referrals and implement Individual Skills Support strategies weekly, maintain daily activity statistics and documentation, and represent the department in multi-disciplinary meetings for planning and review of Individual Support Plans.

Requirements
- Bachelor's Degree in education, psychology, rehabilitation or related field and one year professional experience
- 60 credits toward a Bachelor's Degree with continuing study and 3 years' experience in special education
- Must be 21+ years old, have a High School Diploma or GED, a valid driver's license, daily use of a car on site, and be available by telephone for daily contact
