Role OverviewResponsible for providing care and services of selected member populations across the continuum of illness. Ensures effective utilization and monitors health care resources. Manages the interdisciplinary team to achieve optimal clinical and resource outcomes.
What You Will Do
Manages, coordinates, and facilitates the care delivered to patients through patient/family assessment and multidisciplinary collaboration. Performs daily chart review, utilization reviews, and comprehensive clinical documentation in the electronic medical record.

Requirements
- Current RN licensure in the state of New Jersey
- Bachelor's degree in nursing
- 2+ years of experience in a healthcare setting
- Excellent communication and interpersonal skills
