Job Description
Join the mission-driven team at The Salvation Army Harbor Light Center as we support men on their journey to recovery from substance abuse. The Intake Coordinator will conduct thorough assessments, coordinate services, and provide crisis support, welcoming new clients and connecting individuals to the care and resources they need to succeed.

Requirements

  • Conduct screenings, assessments, and orientations for individuals seeking admission
  • Enter and maintain accurate client data in WellSky and HMIS databases
  • Maintain communication with case managers, probation officers, referral sources, and prospective clients
  • Coordinate with VA case managers to manage logistics and intake for veteran clients
  • Track and manage a list of prospective clients, making referrals to external resources when appropriate
  • Develop and maintain systems to monitor client enrollment, progress, and program outcomes
  • Collaborate with funding agencies to meet intake, assessment, and reporting requirements
  • Prepare reports for contracts with the Department of Human Services and other funding entities
  • Attend staff meetings and collaborate as part of a multidisciplinary team
