Role OverviewThe Intake Coordinator will support Downstate New York referrals from initial intake through the first billable shift, managing coordination and ensuring cases are ready for service.
What You Will Do
The Intake Coordinator will verify Medicaid eligibility, navigate NY Medicaid and payer portals, coordinate caregiver matching and placement, and communicate with various stakeholders.

The ideal candidate will have 1+ years of direct home care experience, Downstate New York home care experience, and experience with NY Medicaid, payer portals, and HHAeXchange or similar home care scheduling/EVV systems.
Requirements
- 1+ years of direct home care experience in intake and/or care coordination
- Downstate New York home care experience
- Experience with NY Medicaid, payer portals, authorizations, and approved service hours
- Home care scheduling, staffing, caregiver matching, and start-of-care experience
- Experience with HHAeXchange or similar home care scheduling/EVV systems
- Bilingual English/Spanish
