Job Description
Center for Elders’ Independence (CEI) provides a Program of All-Inclusive Care for the Elderly (P.A.C.E.) as a holistic healthcare model. The Intake Specialist I (IS I) will be responsible for enrolling and supporting potential PACE participants by navigating the intake/enrollment process, providing program information, and coordinating with various departments. This role involves tracking enrollment, maintaining records, and communicating with healthcare providers and participants. The work area is located at the PACE centers they support, and the IS I acts as a central point of contact for the PACE program.

Requirements

  • Bachelor’s degree in a related field (healthcare, business administration, etc.) preferred.
  • Healthcare experience and understanding of Medicare, Medi-Cal requirements.
  • Excellent customer service and communication skills.
  • Strong case management and self-motivation.
  • Proficiency with computer applications (Salesforce).
  • Valid driver's license and reliable transportation.
