Job Description
The Supported Living Specialist provides leadership and support to employees and individuals served, including the coordination or completion of assessments and development of Individual Support Plans.

Requirements

  • Develop, oversee and modify the initial, annual and all updates to the assessment for each individual
  • Facilitate the development, implementation and on-going follow along of a Person-centered Plan for each individual supported
  • Monitor and ensure the consistency and regulatory compliance of all clinical documents
  • Support the individual’s team around issues, such as, but not limited to: behavioral incidents and crisis management
