Job Description
Wayside Youth & Family Support Network is hiring for a dynamic Intensive Care Coordinator to connect with youth in the field of behavioral health. The role provides care planning and coordination of services for eligible youth under the age of 21 using the Wraparound model.

Requirements

  • Bachelor's Degree in Social Work, Psychology, or related field
  • 1 year of direct experience working with children, young adults, and/or families
  • Valid driver's license and access to a reliable vehicle
