Job Description
The IRIS Consultant provides ongoing support and assistance to participants enrolled in the IRIS program, helping them self-direct services to meet their long-term care goals and life outcomes. This includes home visits, phone contacts, and collaboration with participants, natural supports, and service providers.

Requirements

  • High school diploma or equivalent
  • Four years of experience related to the delivery of social services to targeted groups (intellectual disabilities, physical disabilities, and frail elders) or one year of experience with a bachelor's degree
  • Valid driver's license
  • Vehicle liability insurance
