Role OverviewWe're hiring a Licensed Clinical Social Worker to lead behavioral health intakes and care coordination across a regional network of 8–10 post-acute and long-term care facilities.
What You Will Do
Conduct psychiatric intakes and psychosocial assessments for diagnosis and treatment planning, evaluate patients for eligibility across behavioral health and Value-Based Care programs, and coordinate referrals and transitions of care.
Why It Might Be a Fit
You'll serve as a clinical escalation point for complex cases, mentor and train Behavioral Health Coordinators, and support audits and quality reviews.
Requirements
- Active LCSW license (or equivalent independent clinical licensure) in good standing
- Experience with psychiatric intakes, psychosocial assessments, or behavioral health case management
- Comfort with EHR systems and digital communication tools
- Reliable transportation and willingness to travel regionally
