Role OverviewWe're hiring a Licensed Clinical Social Worker to lead behavioral health intakes and care coordination across a regional network of 8–10 post-acute and long-term care facilities.
What You Will Do
Conduct psychiatric intakes and psychosocial assessments for diagnosis and treatment planning, evaluate patients for eligibility across behavioral health and Value-Based Care programs, and support the clinical development of Behavioral Health Coordinators in the field.
