Serve as the primary point of contact for members, develop and manage personalized care plans, collaborate with healthcare teams, and support members through education and advocacy.

Requirements
- Experience in care or case management (community-based preferred)
- Knowledge of Medi-Cal and ECM strongly preferred

- Willingness to travel within Orange County
- Bilingual (English/Spanish) preferred
