Role OverviewJoin Heritage Health Network as a Lead Care Manager supporting members through the Enhanced Care Management (ECM) program. This hybrid position (2 days remote, 3 days in the field) gives flexibility to work from home while building strong, in-person connections with members across Santa Ana, CA/Orange County, CA.
What You Will Do
Serve as the primary point of contact for members, coordinate their care, develop and manage personalized care plans, collaborate with healthcare teams, support members through education, advocacy, and engagement, identify and address gaps in care, and maintain accurate, timely documentation.
