Job Description
The Lead Case Manager will provide staff support and mentoring to clients using a trauma-informed care, strengths-based approach in community living, and assist clients in developing/maintaining structure in their daily routines to achieve self-sufficiency.

Requirements

  • Supervise Case Managers and interns
  • Provide on-going program evaluation, staff training, and data tracking
  • Model appropriate Case Management techniques
  • Assist with developing and maintaining partnerships with outside agencies
  • Support weekly staff meetings
  • Demonstrate exceptional customer service
