Job Description
The Lead Case Manager will provide staff support and mentoring to clients using a trauma-informed care, strengths-based approach in community living, and assist clients in developing/maintaining structure in their daily routines to achieve self-sufficiency.
Requirements
Supervise Case Managers and interns
Provide on-going program evaluation, staff training, and data tracking
Model appropriate Case Management techniques
Assist with developing and maintaining partnerships with outside agencies
Support weekly staff meetings
Demonstrate exceptional customer service
