Job Description
The Center for Elders’ Independence is a PACE organization providing integrated health care services to the elderly. This role is for a Licensed Behavioral Health Clinician, who will provide diagnostic assessments, treatment, consultation, and crisis interventions for participants in the adult day health center. The position involves collaborating with various team members to develop and implement care plans, participating in interdisciplinary meetings, and facilitating behavioral health psychosocial rounds.

Requirements

  • Master’s degree in Social Work, Marriage and Family Therapy, or Professional Clinical Counselor (LCSW, LMFT, or LPCC license required)
  • 2 years’ experience providing diagnostic assessment, treatment planning, crisis intervention and psychotherapy
  • One year experience working with frail and/or elderly population with Health Psychology model preferred
  • Demonstrated ability to work in a multicultural and multidisciplinary team environment
  • Ability to work independently
  • Excellent oral and verbal communication and meeting facilitation skills
  • Valid California driver's license, acceptable driving record and insurance
  • Reliable transportation
