Role OverviewScreens referrals, coordinates admission process, and provides outreach to community referral sources. Markets area for awareness of SOURCE and PRN meetings. Participates in team meetings and completes paperwork for new admissions and quarterly visits.
What You Will Do
Coordinates services with providers, completes paperwork, and tracks hospital/ER visits. Reevaluates member needs for service and level of service. Uses own vehicle for travel and completes mileage and time sheets for reimbursement.
Why It Might Be a Fit
Requires a current State License as a Licensed Practical Nurse, LPN, and a valid driver's license and automobile insurance liability. Must have a minimum of five (5) years of nursing experience, with hospice or SOURCE care preferred.

- Certified in Basic Cardiopulmonary Resuscitation
