Role OverviewJoin Hill Physicians Medical Group as a Licensed Clinical Social Worker (LCSW) to provide telephonic psychosocial assessments and interventions to assist patients and caregivers in making care transitions and coping with their illness, diagnosis, treatment, and/or life situation.
What You Will Do
Coordinate services, educate patients and caregivers, participate in multi-disciplinary health care teams, assist in advance care planning, and assess and refer for insurance eligibility.

Requirements
- LCSW
- 2-4 years social work experience within the last 5 years (MSW fieldwork not included)
- preferably in a managed care environment

- Salary: $110,000 - $123,000 Annual
