Role OverviewWe are seeking a Licensed Counselor or Social Worker to provide direct behavioral health services to multi-need individuals and families. The role is responsible for developing individualized treatment plans, maintaining compliant documentation, and collaborating with interdisciplinary teams to ensure continuity of care.
What You Will Do
Conduct biopsychosocial assessments, engage individuals and families in the home and community, provide therapeutic and supportive interventions, and develop and update treatment plans using trauma-informed, strengths-based principles.

Requirements
- A bachelor’s degree in counseling, social work or related field is required
- A master’s degree in counseling, social work or related field is preferred
- Active license in the state of Ohio as a Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) is required
- Experience serving multi-need individuals and families is required
- Experience partnering with local behavioral health systems, housing, legal systems, and social services is preferred
