Role OverviewProvide medical social services to PruittHealth Home Health patients and their families in their place of residence. This role involves working with patients and families to assess social service needs, develop plans of care, and provide counseling to assist with coping with significant changes in the patient's condition.
What You Will Do
Complete psychosocial history and assessment, develop plans of care, locate and obtain resources, provide counseling, maintain clinical records, and inform the interdisciplinary team of patient condition and outcomes.
