Role OverviewAs a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes. You are a key part of the care team - delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes.
What You Will Do
Provide direct patient care in accordance with the state Nurse Practice Act, implement and support the plan of care developed by the RN, deliver skilled nursing services, and monitor patient condition.

Requirements
- Active LPN/LVN license in the state of practice
- Graduate of an accredited practical/vocational nursing program
- Current CPR certification
- Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus)
- Strong clinical and observational skills
- Clear communication and patient education abilities
- Ability to follow care plans while recognizing and reporting changes in condition
- Basic proficiency with EMR systems
- Valid driver’s license and reliable transportation
