Job Description
Mentor Community Services provides community-based services for individuals with intellectual and developmental disabilities. The Licensed Practical Nurse will provide prescribed medical treatment and personal care services to individuals under the direction of an RN or physician.

Requirements

  • Associates degree in nursing or related field or state certificate
  • One year experience working with population served in a nursing capacity
  • Current LPN/LVN licensure for state of residence
  • CPR/FA and HIV/AIDS certification, BBP, and ACT training
