Job Description
Achieva is seeking a Licensed Practical Nurse to provide high-quality medical guidance and advocacy for people with disabilities. The LPN will bridge the gap between healthcare and daily support, ensuring that every individual has the health and wellness foundation they need to lead a life of personal significance.

Requirements

  • Graduate of an accredited nursing program
  • Valid Pennsylvania LPN required (BSN preferred)
  • Minimum of two (2) years of experience supporting individuals with intellectual disabilities and medically complex needs
  • Proficiency in computer applications
  • Ability to become proficient in the Therap system
  • Strong organizational, communication, and interpersonal skills
  • Ability to provide constructive feedback and training
  • Reliable vehicle and a valid driver's license for local travel
  • Ability to work flexible hours including evenings and weekends
  • Pass required background checks, including Act 33, Act 34, and FBI clearances
