Role OverviewWe're looking for a Hospice LPN to join our team and make a meaningful difference in the lives of patients and families. As part of an RN/LPN care team, you'll help deliver compassionate, high-quality hospice care to patients living in nursing homes, long-term care facilities, and assisted living communities.
What You Will Do
Provide compassionate, skilled nursing care focused on comfort, dignity, and quality of life. Monitor patients, recognize changes in condition, and communicate promptly with the RN Case Manager and care team.

Requirements
- Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia
