Licensed Practical Nurse (LPN)

Requirements

• Collaborate with and support team members to ensure a patient-focused approach to care
• Provide outstanding customer service while maintaining a compassionate, positive, and respectful attitude toward people who use drugs
• Assess, plan, implement, and evaluate care for patients under their scope of practice
• Collect and record health information findings of physical and developmental assessments into patient record with timeliness and accuracy
• Responsible for screening all new and returning patients by completing an intake process; documents intake encounters and health history into our EMR system
• Facilitate lab and diagnostic tests such as injections, urine and oral toxicology’s at the request of the provider
