Role OverviewJoin our Home Health team as a Licensed Practical Nurse (LPN) to deliver meaningful, one-on-one care to patients in their homes. You'll work closely with an interdisciplinary team to provide hands-on, compassionate care and make a real difference in patients' lives.
What You Will Do
As a Home Health LPN, you'll provide direct patient care, carry out care plans, educate patients and families, document care accurately, and coordinate with the RN and care team.

Requirements
- Active LPN/LVN license in the state
- Graduate of an accredited practical/vocational nursing program
- 3+ years of nursing experience (Home Health, Med-Surg, or Community Health preferred)
- Current CPR certification
- Reliable transportation, valid driver's license, and auto insurance
