Role OverviewAs a Licensed Practice Nurse (LPN), you will deliver skilled and compassionate, one-on-one, patient centered care directly in patients' homes. You will be supported by a mission-driven, nurse-led team that values your contribution and prioritizes patient dignity and outcomes.
What You Will Do
Provide skilled, in-home nursing care to assigned patients, develop and follow individualized care plans, accurately document patient care, observations, and updates, collaborate with interdisciplinary care teams, and maintain compliance with DOL, CMS, and state regulatory requirements.

Requirements
- Current LPN license in the state of WA
- Current CPR certification
- Valid driver's license and reliable, insured vehicle
- Home health experience preferred but not required
