Role OverviewAs a Licensed Practice Nurse (LPN), you will deliver skilled and compassionate, one-on-one, patient-centered care directly in patients' homes, supported by a mission-driven, nurse-led team.

Provide skilled, in-home nursing care to assigned patients, develop and follow individualized care plans, accurately document patient care, and collaborate with interdisciplinary care teams.

Requirements
- Current LPN license in the state of WI
- Current CPR certification
- Valid driver's license and reliable, insured vehicle
- Home health experience preferred but not required
