Role OverviewAs a Licensed Practical Nurse (LPN), you will deliver skilled and compassionate, one-on-one, patient-centered care directly in patients' homes. You will be supported by a mission-driven, nurse-led team that values your contribution and prioritizes patient dignity and outcomes.
What You Will Do
Provide skilled, in-home nursing care to assigned patients, develop and follow individualized care plans, accurately document patient care, observations, and updates, and collaborate with interdisciplinary care teams.

Requirements
- Current LPN license in the state of Tennessee
- Current CPR certification
- Valid driver's license and reliable, insured vehicle
- Home health experience preferred but not required
- Must pass pre-employment screenings
