Role OverviewAs a Licensed Practical Nurse (LPN) at STAT Home Health, you will provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN. You will communicate with attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
What You Will Do
Your essential duties will include providing nursing and supportive care, documenting all services provided, demonstrating knowledge of Medicare/Medicaid regulations, and reporting changes in patient condition and needs to RN.

Requirements
- Current Louisiana LPN license
- Minimum of 2 years as a Licensed Practical Nurse (LPN)
- Home Health experience preferred
- Have dependable transportation, driver's license, and car insurance
