Role OverviewThe LPN Crisis Care Nights will provide crisis care clinical services for patients, visit home and facility team patients as needed, and identify and coordinate patient and family needs.
What You Will Do
Perform ongoing assessment of patient and family needs, take independent action to identify and address issues, and maintain positive working relationships with team members.

Requirements
- Currently licensed in the State of Florida as Licensed Practical Nurse (LPN)
- Must maintain current American Heart Association approved CPR certification
- Minimum of one (1) year nursing experience
- Hospice, Oncology, or Home Health experience is preferred
- Minimum of One (1) year acute care nursing experience with training in Symptom Management
- Most possess and maintain a current Florida driver’s license
- Most possess and maintain automobile insurance coverage
