Paradigm is seeking a fully remote, full-time, LPN Post Acute Referral Care Coordinator to provide staffing for post-acute injured workers, identify healthcare facilities, and coordinate post-acute care services.

Requirements

  • Current LPN state license in good standing with the State Board of Nursing
  • Minimum of one (1) year of nursing experience
