Job Description
We are seeking a Registered Nurse (RN) MDS Coordinator to support regulatory compliance and ensure each resident's clinical and behavioral health needs are accurately reflected. You will complete and transmit MDS assessments, ensure clinical documentation supports resident needs, and partner with interdisciplinary teams to maintain compliance and accuracy.

Requirements

  • Registered Nurse (RN) license in Illinois
  • Current MDS 3.0 certification
  • Minimum of 1+ years of MDS experience in a Skilled Nursing Facility or ICF
