Role OverviewThe MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process, including the Minimum Data Set (MDS), Care Area Assessments (CAA), and care plans in accordance with state and federal regulations.
What You Will Do
Plan, schedule, and complete MDS assessments and submissions, lead interdisciplinary care planning and documentation review, identify and address changes in resident conditions, and monitor Medicare eligibility and documentation.

Requirements
- Current RN license in good standing
- Prior MDS/RAI experience preferred (will train the right candidate)
- Knowledge of state and federal long-term care regulations
- Strong assessment, communication, and documentation skills
